Prescription Products
To obtain prescription products through our Service, you must first complete the full process, which includes reviewing the website information, submitting the Medical Intake, History, and Condition Form, discussing your concerns with the Provider, and signing the Informed Consent. The Provider will assess whether a prescription is appropriate based on the information you provide.
If a Provider issues a prescription, you can choose to fill it through one of our partner pharmacies or any pharmacy of your choice. Prescriptions filled by our partner pharmacies will not use child-resistant packaging. If you opt to use a different pharmacy, we will send you a paper prescription, and you’ll be responsible for fulfilling it and covering the costs.
Our Service is currently available only to individuals in California, Florida, Georgia, Montana, New Mexico, New York, Ohio, and Texas, who are at least 18 years old or of legal age in their jurisdiction. The Service may not be suitable for all medical conditions, and your Provider may advise an in-person visit if necessary.
Third Party Goods and Services
Doc4Life is not accountable for the effectiveness or quality of any third-party products or services, including those offered by pharmacies. Any software or services from third parties that are utilized with our Service are governed by their respective terms and conditions. This Agreement does not establish any rights for third-party beneficiaries.
