Agreement to Use Telehealth Services
Telehealth encompasses the provision of healthcare through digital communications and technology when the healthcare provider and patient are in different locations. While telehealth can offer various benefits, it also comes with its own set of risks. Telehealth services do not replace all in-person care. By using our service, you acknowledge that you have been adequately informed and provide your consent to utilize telehealth services. You agree that telemedicine offers you advantages and necessary access to medication that may otherwise require in-person visits. Doc4Life is recognized as a third-party beneficiary of your consent and retains the right to enforce this agreement with you.

Obligations for Providing Information, Access, and Connectivity
You are responsible for securing and maintaining the necessary software and hardware as required by the Service. This includes ensuring you have a computer or mobile device with internet access and adequate privacy settings. Any expenses related to your internet or mobile connections are your responsibility.
You must provide accurate, complete, and truthful information in any forms or communications submitted through the Service. We reserve the right to modify access configurations, including software or hardware requirements, without prior notice.

Financial Responsibility for Services
Doc4Life does not participate in federal or state healthcare programs like Medicare, Medicaid, or Tricare. Consequently, neither Doc4Life nor its Providers will seek or receive payments from such programs for services provided through our platform. Services obtained through Doc4Life are generally not covered by federal or state healthcare programs. By using our services, you agree to pay out-of-pocket and assume full responsibility for any costs.

Registration, User Accounts, and Security
To access the Service, you must create a registered account with a username and password. Using another person’s account is prohibited.
You must keep your information up-to-date and accurate. Failure to do so, or providing false information, may result in the suspension or termination of your account. You must promptly notify Doc4Life of any unauthorized use of your account details by emailing support@doc4life.online.
Keep your username and password private and ensure you log out after each use. Doc4Life is not responsible for any losses that may result from your failure to protect your account information. The security of your account relies on how well you secure your devices and login details.

Service Use and Ownership
The Service and its content are protected by copyright laws. Doc4Life grants you a temporary, limited, non-transferable, and revocable license to use the Service for personal purposes. All rights to the Service and its content are reserved by Doc4Life. You may not alter or obscure any proprietary notices associated with the Service. Doc4Life’s trademarks and logos are protected and cannot be used without permission

License for Submitted Information
Information you provide to Doc4Life (referred to as “Submissions”) is considered non-confidential unless restricted by privacy laws. You grant Doc4Life a broad, perpetual, and royalty-free license to use, distribute, and display your Submissions as needed to operate and provide the Service. You remain responsible for your Submissions and agree to waive any moral rights associated with them.

Prohibited Activities
The Service must not be used for any illegal or harmful purposes. This includes, but is not limited to, damaging servers, accessing unauthorized systems, uploading malicious content, or breaking laws. Additionally, creating fake identities, sending unsolicited advertisements, and infringing on others’ rights are also prohibited.
Doc4Life retains the right to take appropriate legal actions in response to these violations, which may include suspending or terminating access to the Service. We may also collaborate with law enforcement in the investigation of suspected illegal activities.

Copyright Policy
Doc4Life will terminate accounts of users who repeatedly infringe on copyrights. If you believe your copyrighted work has been infringed, please provide a notice to our Copyright Agent at support@doc4life.online with detailed information, including your contact details and a declaration under penalty of perjury.

Terms of Sale
Product availability through the Service is subject to change. We reserve the right to limit quantities or reject orders. If an error occurs, we may correct it and adjust your order or issue a refund. Prices may change, but this will not affect existing orders. You are responsible for any applicable taxes. You will use the payment methods that we provide, and by completing a purchase, you authorize us to charge your selected payment method. All payments are processed by an external payment service provider, and Doc4Life is not responsible for any issues or errors that may occur during the transaction process.

Termination
Doc4Life may terminate your use of the Service at any time for any reason, including violations of this agreement. Upon termination, you may not attempt to use the Service under any name. We have no obligation to return or provide any information or content after termination.

Notices
Any notices from Doc4Life regarding the Service or this agreement may be communicated via email, posted notices on the Service, or regular mail.

Electronic Communications
By using the Service or sending emails to us, you consent to receiving communications electronically. All agreements and notices provided electronically satisfy legal requirements for written communication.

Disclaimers
The Service is offered on an “as is” basis and you use it at your own risk. Doc4Life and its third-party partners disclaim all warranties, including any implied guarantees of merchantability or suitability for a specific purpose. We do not ensure that the Service will be free from interruptions, delays, or security issues. 

Limitation of Liability
To the fullest extent permitted by law, Doc4Life and its third-party providers are not liable for any incidental, indirect, or consequential damages arising from your use of the Service. Our liability is limited to actual damages not exceeding $1,000. Claims must be brought within one year, and participation in class action lawsuits is waived.

Indemnification
You agree to indemnify and hold harmless Doc4Life and its partners from any claims or damages arising from your use of the Service or any violations of these Terms.

Entire Agreement
This agreement constitutes the entire understanding between you and Doc4Life regarding your use of the Service and supersedes any prior agreements.

Binding Arbitration
Any disputes arising from your use of the Service will be resolved through binding arbitration, not in court. Arbitration will be conducted in Sheridan Wyoming and must be on an individual basis only, with no class or collective actions. You can opt-out of arbitration provisions within 30 days of agreeing to this agreement by sending a notice to Doc4Life.
